You can use this form to apply for assistance with transport to school or college for eligible students. We ask for parent or guardian bank details but payment will only be taken if the application is successful.

We currently only accept payment online by direct debit. You must be the account holder of a UK bank or building society and be the only person required to authorise payments from this account. View the alternatives to paying online by direct debit.

Before you complete this form, please make sure you have to hand:

• details of the sixth form or college you wish to travel to;

• full details of the course you are planning to attend;

• parent or guardian bank account details; and

• electronic copies of working tax credit or child tax credit, where applicable.

Once you have this information, it should take approximately ten minutes to complete this form. There are six pages in total.
